Referral Partner Hub

Refer a participant safely, and know exactly what happens next.

You should be able to understand the service, know what information is needed, submit it through an appropriate pathway, and receive status back. This page sets that expectation in writing.

Privacy design rule

Our general contact form is intentionally separate from the referral workflow. Please do not send diagnoses, Medicaid ID numbers, service plans or other protected information through a public web form or a generic email inbox. Call us and we will route you to the appropriate secure pathway.

Secure referral portal

Role-based access with an audit trail, set up per referring organization. Contact our team to be onboarded and to receive the referral toolkit for your staff.

Referral toolkit

Short one-page summaries your staff can use at the point of referral: service description, service area, diet and texture pathways, what information to collect, and expected turnaround. Ask us to send the current version — we keep it updated rather than posting stale documents.
